Should You Go With the AMS 700 or the Coloplast Titan?

There are multiple manufacturers of penile implants, and choosing the right one for you is based on your unique physiology and anatomy. This guide can help with your decision-making, and your urologist can help you fill in any blanks.

Penile implants.

There are plenty of visceral reactions at the mention, but for many (96%) men and their partners (90%), the feedback is consistent.

“GAME CHANGER!”

“I should have done this YEARS ago”!”

“It brought my confidence back.”

“It changed our lives.”

Many guys will experience erectile dysfunction (ED) at some point in their lives, whether it’s due to age, medical conditions like diabetes or prostate cancer, or other factors, and implants are a reliable solution that restores predictability in sexual function. While not typically a first-line solution, a penile implant is a reasonable long-term one when medications or other treatments stop working. Two of the primary inflatable implantable penile prostheses on the market are the Boston Scientific AMS 700 and Coloplast Titan. Both are designed to achieve the same goal, but when it’s time to make that decision, which one should you pick?

Boston Scientific AMS 700

The AMS 700 is one of the most widely used inflatable penile implants and comes in several variations to better match a patient’s natural anatomy.

Made from a soft, layered silicone material, the AMS 700 is designed to provide a natural feel when the implant is inflated or deflated. After healing, many patients and their partners report that the device is not noticeable during regular daily activity or intercourse. So, how is the rigidity with AMS 700? Rigidity is not determined by the implant alone, although it is a factor. The health of surrounding penile tissues (the tunica albuginea, a fibrous outer layer that helps maintain firmness and underlying vascular tissue) is fundamental to how firm an erection feels. The AMS 700 provides excellent rigidity for intercourse in the vast majority of cases, especially when those tissues are uncompromised and structurally sound.

The cylinders are part of the implant, actually placed inside the penis, and filled with fluid to create an erection. Sizing is based on internal measurements taken during surgery, with the focus on proper fit rather than enlargement or enhancement. The AMS 700 comes in a range of cylinder sizes to accommodate different penile lengths and widths. The AMS 700 CX model allows for controlled girth expansion over time, while the AMS 700 LGX model allows for both girth and length expansion. While this may seem like an appealing factor to most patients, the keyword is “controlled.” In other words, the change is gradual and may be 1-2 inches in length.

The implant includes a fluid reservoir that is placed in the abdomen such that it isn’t visible or noticeable. In some cases, a lower-profile version can be used when an individual’s anatomy requires it.

In order to inflate the device, a small pump is placed in the scrotum. With a few squeezes, the device is inflated manually and deflated using a release mechanism. One feature of this system is a deflation button that can be activated with a single press, rather than requiring prolonged pressure. As fluid moves out of the device during deflation, patients may feel a slight vibration or feedback, which can help confirm that the process is working. There is also a lock-out feature designed to prevent unintended inflation.

The AMS 700 comes with a factory-applied antibiotic coating called InhibiZone™, which helps reduce postoperative infection risk, as demonstrated by clinical data showing a reduction in revision surgery related to infection.¹

Coloplast Titan

Like the AMS 700, the Coloplast Titan comes in different sizing options to match a patient’s anatomy. Both restore erectile function, and both are placed internally with a pump in the scrotum and a fluid reservoir in the abdomen.

Made with a stronger material known as Bioflex (a proprietary blend using polyurethrane), the Titan is designed to withstand higher internal pressures when inflated and can feel slightly firmer when fully activated with more rigidity for “thrust”.. The trade-off is that, when deflated, the material can sometimes “dog-ear,” or fold in a way that is more noticeable under the skin. It isn’t a functional issue and does not affect sexual performance. For most, a few pumps of fluid also eliminates the “dog ear” appearance.

Rigidity is influenced by the same underlying factors as other implants: the Titan is designed to provide strong rigidity when inflated, and most patients achieve excellent firmness for sexual activity when tissue health is good. The Titan has a leg up with a range of standard and narrower cylinder options that address broader anatomical variations. That sort of flexibility in sizing can be helpful in patients with more complex anatomy or scar tissue, particularly in patients with Peyronie’s disease or severe penile fibrosis.

The fluid reservoir differs from the AMS 700 in its cloverleaf design. It’s still placed in the abdomen, but the reservoir shape of the Titan is intended to help with placement in different anatomical spaces.

When inflating the Coloplast Titan device, some patients find it easier to control, particularly if hand strength or dexterity is limited. The pump simply requires a clear, deliberate squeeze to activate inflation and deflation. A built-in lock-out feature in the reservoir also helps prevent accidental inflation.

The Coloplast Titan uses a different postoperative infection-reduction approach with a hydrophilic coating called HydroVANTAGE™. The coating absorbs an aqueous antibiotic solution selected by the surgeon immediately before implantation. Antibiotic coverage can be tailored to the patient and is especially useful in patients with allergies or sensitivities to the antibiotics used in InhibiZone™ coatings.²

The longevity and durability of any implanted medical device depend on individual anatomy, usage patterns, follow-up care, and overall health. More frequent or vigorous sexual activity may contribute to wear, but most patients with typical use patterns can expect these implants to function well for 10 – 15 years.

Both the AMS 700 and Coloplast Titan are manufactured by well-established companies here in the US with reliable safety standards and quality control; device-related mechanical failures are uncommon in either one. The differences are more related to feel, pump mechanics, and anatomical fit rather than overall effectiveness.

Rather than making a decision based on brand, it’s best to discuss the best device for an individual’s physical size and rigidity preferences. Some additional details related to sexual activity might also be part of the conversation, from a medical and safety standpoint, to suit the patient’s needs.
If you’ve tried medication and are ready to explore the next step, the team at Georgia Urology is available to answer questions and help determine whether a penile implant is appropriate for your situation.
